With our plugin you can easily link forms from the form manager with a shipping method and display them after each order. Possible options would be e.g. pickup time or surveys.

About the Extension
This plugin allows you to display any form from the Form Manager on the order completion page. After completing an order, your customers can, for example, fill out a feedback form or answer the question about a specific collection date.
To use this function, create a form as usual. This is done via the form manager in the backend of your shop, which you can reach via the path Contents → Forms.
You can then select the appropriate form for each shipping method in the shipping costs administration (Shipping costs → settings).
The forms are always sent asynchronously (AJAX) so that your customers remain on the order completion page and are not forwarded.
