Description
Capture additional information directly at the order confirmation step – flexible, structured, and without any template customization.
With the Checkout Form Manager, you can extend the order confirmation page in Shopware 6 with custom content and form fields – no individual template adjustments required.
All elements are managed centrally in the administration and can be placed at specific positions within the order confirmation page, e.g. below headings or after the terms and conditions section.
Flexible Form Fields
Create custom input fields to collect additional information directly during the order completion:
- Single-line and multi-line text inputs
- Dropdown fields
- Checkboxes
- File uploads (e.g. images or documents)
Each element can optionally be set as required, supports placeholders, and allows preselection using Twig syntax (e.g. to automatically prefill customer data).
Structure & Content Flexibility
In addition to form fields, you can use various elements to structure and display content:
- Headings
- Spacers
- Custom HTML content
- Shopware standard notifications (e.g. info or success messages)
This allows you to enhance your order confirmation page not only functionally, but also visually and structurally.
Flexible Placement
The order confirmation page is divided into multiple sections. You can decide exactly where your content should be displayed for maximum flexibility.
Storage & Display
All entered data is stored directly with the order and is available in the administration within the order details.

Setup Guide
- Install the extension
- Configure the extension according to your needs
- In the administration, navigate to "Settings" → "Extensions" to find the new menu item "Checkout Form Manager"
- Select the desired section in the left sidebar
- Click "Add element" to create new form fields
- The new fields will be immediately visible on the order confirmation page
