Create forms, place them where they matter and keep submissions traceable - directly in the Shopware administration.
Form Manager turns individual request forms into a central form system for your shop. You build forms in the administration, structure them with groups and rows, add input fields, notices, uploads and conditions, and place them where the request happens: in Shopping Experiences, on product detail pages or in the order area of the customer account.
Why use a dedicated form manager?
A simple contact form is often enough for the first step. In daily shop operations, however, more use cases appear quickly: quote requests, callbacks, claims, product questions, uploads or order-related follow-up questions. If every form is solved separately through email, CMS blocks or individual templates, maintenance becomes scattered and evaluation becomes difficult.
Form Manager brings these processes together. New forms are created in the administration instead of in templates. Fields can be grouped logically, arranged in multiple columns and displayed depending on previous inputs. After submission, the data remains traceable in the shop and can, depending on the enabled feature set, be filtered and exported.

Structured forms instead of flat field lists
Each form is built as an element tree in the administration. Groups bundle related sections, rows allow multiple elements next to each other and field widths follow a 12-column grid. This keeps even larger forms understandable and prevents the storefront from feeling like a random list of inputs.
In addition to classic inputs such as text, select, checkbox, radio, email or phone, the plugin supports uploads, image uploads, date, time, date with time, number, slider, color picker, URL, country, consent, salutation and combined contact fields. Additional content elements such as headings, separators, free text and Shopware-style alert messages help make forms easier to understand.
Dynamic forms without code
Conditions make forms more flexible. A field can appear only when a previous selection was made, when a text contains a value, when a value is empty or when a number or date falls within a specific range. Multiple rules are handled as OR conditions: one matching rule is enough to show the element.
This logic does not only run in the browser. On submit, the server evaluates the same conditions again, validates only visible input fields and stores only actual input values. This keeps submissions clean and avoids storing data for fields the customer should not see or fill in.
Integration in storefront, products and orders
Forms can be used as Shopping Experience elements and placed on content pages, landing pages or other CMS areas. For product-related requests, central assignments are available: choose a form, individual products or a dynamic product group, optionally a Shopware rule and a priority. Maintenance stays in one place instead of being spread across product fields.
Forms can also be used in the order area, for example for follow-up questions, service cases or additional information about an order. Global switches and display options control whether these forms are shown directly, collapsed or as a link.
History, email and CSV export
After submission, an email notification can be sent to the shop operator. In addition, submissions are stored in the shop. The history shows form, context, customer, sales channel and date; search and filters help narrow down entries.
The CSV export is based on the stored historical submission data. This means older submissions generally remain evaluable even if a form is changed or deleted later. Structure elements such as groups or notices are not stored in the payload; the export contains the actual input values.
Close to Shopware standards
Form Manager uses existing Shopware concepts instead of building isolated parallel solutions. File and image uploads are stored in the Media Manager, product and order assignments can be combined with the Rule Builder and optionally the standard CAPTCHA configured in Shopware is validated. Honeypot, Basic CAPTCHA and Google reCAPTCHA v3 are supported.

Form Manager is useful for contact and consultation forms, quote and B2B requests, callbacks, product questions, service cases, claims with uploads and order-related communication in the customer account. It is especially helpful when a form needs more context than just name, email and message.
